AI/ML medical devices are characterized by continuous iteration and ongoing model optimization. Under the traditional regulatory framework, every modification that may impact safety and effectiveness requires a new premarket submission, which is time-consuming and costly, severely slowing down the rollout of algorithm iterations. To resolve this conflict, the FDA launched the Predetermined Change Control Plan (PCCP), opening a compliant post-market modification pathway for medical device manufacturers. Today, combining official FDA resources, we break down the underlying logic, core content and applicable boundaries of PCCP.

Impacts of individual modifications and combined multiple modifications; Cascading effects triggered by one modification on other planned changes; Benefits and risks associated with each category of modification; Evidence demonstrating that this protocol can continuously ensure the safety and effectiveness of the medical device.
| Dimension | Traditional Post-Market Modification | Predetermined Change Control Plan (PCCP) |
|---|---|---|
| Authorization Timing | After device clearance/approval; re-evaluate whether a supplemental submission is required once changes occur | Authorized in advance during the original premarket submission stage |
| Submission Logic | After the change occurs, judge risk and submit new/supplemental applications case-by-case | When changes roll out, execute per the predefined protocol; no repeated submission for in-scope modifications |
| Binding Terms | Follow post-market modification rules and submission obligations | All revisions must strictly comply with the PCCP document; out-of-scope changes still require new submissions |
Note:PCCP shall be arranged as an independent section within the submission package and highlighted in the submission cover letter. In the eSTAR electronic submission template, select the corresponding checkbox under the General Device Characteristics section and upload attachments.
